Firefighter Wellness Workshop Follow-up

This membership rate and these services are only available to Firefighters who have participated in the Holistic Health & Wellness for Firefighters workshop. Firefighters must sign-up for the membership or services within 365 or less of completing the workshop to be eligible for these rates. All Firefighter Workshop Participants are eligible for 1x ~5-10 minute follow-up phone call at no cost within 365 days of the session to discuss any questions or concerns stemming from the session.

Firefighter Telehealth Medical Membership

This 12-month membership is available for Firefighters in Ohio or Michigan. This package includes:

Reserved spot as a client, guaranteeing access to services.
Initial introduction, fit, & needs assessment virtual meeting (~30-40 min.).
Direct text/email access for brief, basic health or wellness questions (limited to 20 per year or ~ 1-2 questions per month)

Direct physician health issue triage for minor or chronic health issues.
Access to Medical Member Only Video & Digital services and packages (excludes In-person Meeting & House Call services).

Discounted pricing of 75% off all Video & Digital Medical & Non-medical services & packages.

Priority scheduling for all services & packages.

In-Depth Health Screening & Preventive Care Consultation

Video / Phone Meeting

This session provides an opportunity for an in-depth health screening & preventive care planning discussion. The session includes:

Review and discussion of individual exposure risk factors, family medical history, vaccination schedule modification risk factors,

Evaluation and discussion of personal vaccination, preventive health screening, and cancer screening needs.
Evaluation and discussion of responses to GAD-7 & PHQ and other mental health needs.

Evaluation and discussion of any Relational, Sexual, or Professional Wellness concerns or challenges.
Discussion of any clarification questions or general concerns related to Existential Wellness. (Full discussion of advanced care planning and existential philosophy takes extended time and is offered separately.)

Guidance on how to have your personal vaccination, preventive health, and cancer screening service needs met.

This session is intended to be primarily educational and coaching in nature. Lab testing, imaging, and other diagnostics are not ordered as part of this session. No medications will be prescribed.

Personal Emergency Preparedness Planning Consultation

Video / Phone Meeting

This session provides education and guidance to help Firefighters have the best possible outcomes if personal health emergencies arise. The session includes:

Discussion of personal values, philosophies, & spirituality that guide personal health and wellness decision-making.
Discussion of personal medical issues and concerns.
Evaluation of personal medical risk factors.
Education on emergency & hospital-level medical treatment and processes.

Education and counseling on organizing critical health information to promote safer hospital admissions.

Discussion of preferred care, support, and decision-making people, including unique relationship or family dynamics.

Discussion of key planning and documentation tools (e.x., code status, living willings, medical power of attorney, advanced care directives, etc.)

Counseling and guidance on strategies for increasing the likelihood of obtaining values & goals-aligned emergency care.
Development of a personal emergency documentation packet (follow-up communication for initial development included).
